RELEASE FROM LIABILITY
Dear Special Event Applicants: On advice of the city's insurance provider, the city is requiring that any organization applying for a special event in the city include a hold harmless clause in their specific event registration (online form or paper) that each vendor, participant or organization acknowledge or sign that indemnify and hold harmless the City of Clawson moving forward. The language is listed below: "HOLD HARMLESS CLAUSE To the fullest extent permitted by law I, agree (Name of Organization, Company, Participant etc.)to defend, pay on behalf of, indemnify, and hold harmless the City of Clawson, its elected and appointed officials, employees and volunteers, and others working on behalf of the City of Clawson against any and all claims, demands, suits, or loss, including all costs connected therewith, and for any damages which may be asserted, claimed, or recovered against or from the City of Clawson, by reason of personal injury, including bodily injury or death and/or property damage, including loss of use thereof, which arises out of, or is in any way connected or associated with this contract. PET CERTIFICATION
l attest that my dog is properly licensed and has received all required vaccinations including those for Rabies. I shall have available, and provide upon request, documentation to validate this assertion. I acknowledge that female dogs in any state of heat will not be allowed to participate, and I will voluntarily comply with this requirement. I under-stand that event officials have the discretion to request that pets whose behavior is unruly or might present a hazard to other event participants, other dogs or spectators leave the event, and I will immediately comply with such a request. / will keep my dog on a short leash at all times and will toke appropriate measures to control my dog at all times, giving consideration to fellow participants. I agree to be personally responsible and liable for the behavior and actions of my dog. Choke chains/choke collars not permitted.
